Insurance
All of our providers accept self-pay, and some providers also accept Anthem Blue Cross/Blue Shield, Aetna, and Caresource Marketplace.
Dr. Pratt is in-network with Anthem Blue Cross Blue Shield, Aetna, and Caresource Marketplace until May 31, 2024. She will be out-of-network effective June 1, 2024.
Please contact your insurance carrier to ensure that your clinician is an in-network provider. We are happy to provide documentation for you to seek out-of-network reimbursement if your provider is not covered by your insurance plan.

The following fees are listed for those who do not have insurance coverage or choose to not utilize their insurance.
These fees do not reflect co-pays one may owe with insurance.

If you need to cancel your appointment, please provide us with at least notice 24 hours in advance. A no-show fee of $75 will be automatically charged if you do not arrive for your scheduled appointment or do not give us at least 24 hours notice.
Non-Covered Services
*Non-Covered Services include all services not typically covered by third-party payers (mental health/medical insurance). Examples include, but are not limited to, completion of paperwork for disability claims, FMLA documentation, review of records, and consults with other professionals.
A quote will be provided and agreement for payment of services rendered will be required before services will be provided.
